IMO when USSTRATCOM makes the following statement:

"USSTRATCOM is primarily interested in the active satellites, but also tracks space debris. The SSN tracks space objects that are 10 centimeters in diameter (baseball size) or larger."

it is pure fiction. They may have the capibility but IMO they do not have the interest in tracking everything. The Sirius 3 Rocket is one example, the MDS 1 launch last year is another example (http://satobs.org/seesat/Sep-2002/0181.html) and it now appears that there was evidence of a piece of debris near Columbia just after launch that went uncatalogued (http://www.washingtonpost.com/wp-dyn/articles/A2316-2003Feb25.html).
